It measures the activity of alkaline phosphatase, an enzyme present in white blood cells, particularly neutrophils. In CML, the LAP score is typically low or absent, while in leukemoid reactions, it is elevated. By analyzing the LAP score, healthcare professionals can make a distinction between these two conditions. CML is a myeloproliferative neoplasm characterized by the presence of the Philadelphia chromosome and the BCR-ABL1 fusion gene. Leukemoid reactions, on the other hand, are reactive responses of the bone marrow to various stimuli such as infections or inflammatory conditions. The clinical utility of the LAP Score lies in its ability to aid in the differential diagnosis and management of patients presenting with leukocytosis, helping to distinguish between CML and transient leukemoid reactions.
