Sputum Test Overview
Tuberculosis is a chronic respiratory tract infection causing fever, cough and weight loss. It is commonly caused by Mycobacterium tuberculosis (M.tb). Many diagnostic techniques are available for diagnosis of tuberculosis which include staining, culture and nucleic acid detection. Among nucleic acid detection techniques are polymerase chain reaction (PCR), Cartridge based nucleic acid amplification technique (CBNAAT), Line probe assay (LPA), Next generation sequencing (NGS).
LPA is based on DNA strip technology. It is approved by WHO for testing pulmonary samples (smear positive) and M.tb culture isolates. It tests for rifampicin and isoniazid resistance.

The test is done to diagnose drug-resistant TB. The samples that are smear-positive for AFB are tested again by LPA. It enables rapid drug-susceptibility testing for drugs like rifampicin and isoniazid. It detects DNA of M.tb that causes resistance to rifampicin and isoniazid.
Sputum test detects DNA of Mtb and mutations which can lead to Rifampicin and Isoniazid resistance
During sputum test patient will be advised to rinse the mouth with clean water and then expectorate sputum with a deep cough. Aim is to get secretions from the lower respiratory tract which are not contaminated with sputum. Sputum sample will be collected in a screw capped container.
Test can be booked if pure culture of M.tb is provided at the time of booking
M.tb detected - This means that the DNA of M.tb is present in sample and patient is likely to be suffering from tuberculosis.
M.tb Not detected - This means that the DNA of M.tb is absent in sample and patient is unlikely to suffer from tuberculosis.
Resistance (mutations) to Rifampicin, Isoniazid (KatG), InH A promoter region are reported by the test
If resistance to fluoroquinolone is detected, it means that the patient may not respond to treatment with fluoroquinolones.
If resistance to aminoglycosides is detected, it means that the patient may not respond to treatment with aminoglycosides.
There are no special preparations needed for the test.
No, sputum is collected consecutively for three days to ensure enough bacteria in the sample.
It is generally collected in the early morning before breakfast.
The healthcare provider may ask the patient to breathe in saline mist. This leads to intense coughing. If the patient is not able to cough up enough sputum even after breathing in a saline mist, a bronchoscopy may be recommended to collect samples
