Sex hormone binding globulin (SHBG) is a protein produced by the liver that transports the hormones testosterone which is an androgen, dihydrotestosterone (DHT) also an androgen, and estradiol which is an estrogen in the blood as which are traditionally, inactive. Modulations in SHBG levels can affect the amount of hormone that is available to be utilised by the body's tissues.

About 45 to 60 per cent of testosterone in blood normally amount to SHBG in men and only 2 to 3 per cent of testosterone is readily available to use.  While in women, SHBG plays a critical role in regulating the levels of androgens and estrogens circulating throughout the body.

If your SHBG level is not normal, it can affect the level of hormones in men and women leading to concerns like infertility, erectile dysfunction and irregular periods.

This test is done if there are symptoms of excessive or decreased androgen activity & is also useful in the evaluation of mild disorders of androgen metabolism. Symptoms include decreased sex drive, depression and anxiety, reduced muscle and bone mass and other symptoms.
 

The test measures the levels of SHBG in the blood. 

The test requires a blood sample. A tourniquet (elastic) band is placed tightly on the upper arm. The patient is then asked to make a fist. This helps in the build-up of blood filling the veins. The skin is disinfected before needle insertion and the blood sample is collected in vacutainer. 

High values are seen in hyperthyroidism, hypogonadism, androgen insensitivity and hepatic cirrhosis in men.
Low concentrations are seen in myxoedema, hyperprolactinaemia and syndromes of excessive androgen activity.
