Thyroperoxidase (TPO) is an enzyme involved in thyroid hormone synthesis. 
Thyroid peroxidase antibodies (TPO) antibodies destroy cells of thyroid gland and cause hypothyroidism.
Thyroglobulin (TG) is a protein formed in thyroid gland and serves as a substrate for synthesis of thyroid hormones. Thyroglobulin antibodies (ATA) thus prevent formation of thyroid hormone leading to hypothyroidism. These antibodies are hallmark of Hashimoto thyroiditis but can be seen in Graves' disease also.

Thyroid antibodies test measures the level of ATA, TPO antibody in blood

Thyroid antibodies test requires a blood sample. A tourniquet (elastic) band is placed tightly on the upper arm. The patient is then asked to make a fist. This helps in the build-up of blood filling the veins. The skin is disinfected before needle insertion and the blood sample is collected in vacutainer.

The Thyroid Antibodies (TPO) and ATG Serum tests detect the TPO and ATG antibody levels in the patient’s serum. High levels of these antibodies may indicate the presence of a thyroid disorder.

Normal levels : no thyroid antibodies were found. This means that symptoms of hypothyroidism are probably not caused by deficiency of TPO or ATA.
High values: suggestive of  Hashimoto thyroiditis; these antibodies can also be seen in Grave's disease

If the results of the Thyroid Antibodies (TPO and ATG Serum) tests are present, it may indicate an active thyroid disorder. If the results are absent, it may indicate the absence of a thyroid disorder.

Thyroid antibodies (TPO and ATG serum) should be prepared according to the laboratory's instructions. Generally, the patient will be asked to fast for 8-12 hours before the test. He will also be asked not to take any thyroid medications for at least 48 hours before the test.

The normal levels of TPO and ATG antibodies should be below 10 IU/mL. A result that is higher than 10 IU/mL indicates the presence of a thyroid disorder.

Patients should speak with their healthcare provider to determine if this test is right for them. Generally, this test is recommended for those:

  • With known or suspected thyroid disorders such as Hashimoto’s thyroiditis
  • Who have had a previous episode of hypothyroidism
